How does Triptorelin work?

Triptorelin binds GnRH receptors on the pituitary. The first exposure causes a big release of LH and FSH (the 'flare'), which pushes the testes to make more testosterone for a few days. With continuous exposure, as from a depot injection, the receptors desensitize and LH and FSH collapse, driving testosterone down to castrate levels within about 2 to 4 weeks. That is the whole point of the prostate cancer use. The single-dose restart idea tries to use only the flare: one short-acting dose stimulates the pituitary without enough ongoing exposure to shut it down. The catch is that the formulation matters enormously. A depot dose of triptorelin is designed to suppress testosterone for 1 to 6 months, which is the opposite of what someone trying to recover wants.

What does the research show?

For the approved uses, the evidence is solid. Trelstar labeling shows testosterone rising for the first few days, peaking on days 2 to 4, then falling to castrate levels (under 50 ng/dL) by weeks 3 to 4 in men with prostate cancer, with effects usually reversible after stopping. Triptodur is approved for central precocious puberty in children 2 and older. A 100 mcg triptorelin stimulation test is also an established diagnostic tool: a Chinese study of 229 males found LH peaked 45 to 60 minutes after the injection and that the peak level helped separate delayed puberty from hypogonadotropic hypogonadism. The HPTA restart use is much thinner. The best-known source is a 2010 Fertility and Sterility case report of one 34-year-old man with hypogonadism after internet-bought anabolic steroids: he got a single 100 mcg triptorelin test dose and his testosterone was back in the normal range within a month. That is one patient, no control group, and the dose was given as a diagnostic test, not as a treatment. A 2023 scoping review in Endocrine Connections found that gonadotropins usually recover on their own within 3 to 6 months after steroids stop, while testosterone recovery is often incomplete, and it did not identify triptorelin as a studied recovery treatment. Bottom line: plausible, cheap, and popular, but not proven to beat natural recovery or standard options.

What doses have been studied?

FDA Approved·FDA approved for prostate cancer and precocious puberty; restart use rests on one case report

Typical Dosingⓘ

Community experience

Common Dose

Depot per label (3.75-22.5 mg); 100 mcg single dose for restart (community-reported)

Range

100 mcg single dose (stimulation test) to 22.5 mg depot every 24 weeks

Frequency

Single dose for the stimulation test; every 4, 12 or 24 weeks for depots

The restart use relies on the short-acting acetate form at 100 mcg once, mirroring the diagnostic stimulation test. Depot products (Trelstar, Triptodur, Decapeptyl depots) suppress testosterone for months and are the wrong tool for recovery. Blood work before and after is the only way to know whether it did anything.

Research Dosingⓘ

Scientific studies

FDA-approved depot dosing, plus the diagnostic test dose used in published case and cohort reports

Duration

Ongoing for approved depot uses; single injection for the stimulation test

Administration

Intramuscular injection (depots); subcutaneous or intramuscular for the 100 mcg short-acting dose

When and how is Triptorelin taken?

Best Time to Take

Morning for the single 100 mcg dose; depots on the scheduled date

Once (stimulation or restart dose) or per depot schedule

Food Recommendation

With or without food

Why This Timing?

The stimulation test is usually done in the morning so LH can be measured 45-60 minutes later. Depot timing only needs to follow the 4, 12 or 24 week schedule.

What are the side effects of Triptorelin?

Not everyone experiences these effects. Individual responses vary based on dosage, duration, and personal factors.

  • ●Hot flushes (very common with depot use)
  • ●Erectile dysfunction, low libido and testicular atrophy with ongoing suppression
  • ●Temporary testosterone flare in the first days (can worsen prostate cancer symptoms)
  • ●Prolonged testosterone suppression if a depot form is used by mistake
  • ●Headache and skeletal pain
  • ●Injection site pain
  • ●Raised blood sugar, diabetes and lipid changes with long-term use
  • ●Increased cardiovascular risk and QT prolongation with long-term androgen deprivation
  • ●Hypersensitivity reactions, including anaphylaxis (rare)
  • ●Convulsions (rare)
  • ●Severe skin reactions such as Stevens-Johnson syndrome (rare, added to US label 2025)

Frequently Asked Questions

What does Triptorelin do?

Triptorelin is a synthetic analog of gonadotropin-releasing hormone (GnRH) with a D-tryptophan swapped in at position 6, which makes it far more potent and longer-lasting than the natural hormone. As a long-acting depot it is FDA-approved to shut down testosterone in advanced prostate cancer (Trelstar) and to pause early puberty in children (Triptodur), and in Europe it is sold as Decapeptyl, including a short-acting 0.1 mg form used in IVF. In men's health circles it is known for something else: a single small dose used to 'restart' testosterone after steroid use. That idea rests on very little published evidence.

Related Peptides

Peptides commonly compared with Triptorelin or used in similar applications.

Leuprolide

FDA

Leuprolide (brand name Lupron) is a synthetic GnRH agonist, a modified peptide far more potent than natural GnRH, and it is FDA-approved for advanced prostate cancer, endometriosis, uterine fibroids, and central precocious puberty. The clever part is counterintuitive: it works by overstimulating the GnRH system so hard that the pituitary shuts down sex hormone production. It is a real, long-established prescription drug (first US approval 1985), not a research peptide, and it carries meaningful side effects from the low-hormone state it creates.

Hormonal

Octreotide

FDA

Octreotide is a synthetic eight-amino-acid mimic of the natural hormone somatostatin, the body's main 'off switch' for hormone secretion. It shuts down excess growth hormone, so it is a frontline FDA-approved drug for acromegaly, and it also tames the flushing and diarrhea of hormone-secreting carcinoid and other neuroendocrine tumors. This is real, approved medicine with decades of clinical data behind it, sold as Sandostatin (injectable, since 1988), Sandostatin LAR (monthly depot), and Mycapssa (oral capsule, approved 2020).

Hormonal

Lanreotide

FDA

Lanreotide (development code BIM-23014) is an eight-amino-acid somatostatin analog, a close cousin of octreotide, given as a long-acting deep-injection gel (Somatuline Depot/Autogel) usually once a month. The FDA approved it for acromegaly in 2007, for gastroenteropancreatic neuroendocrine tumors in 2014, and for carcinoid syndrome in 2017. Generic versions now exist alongside Ipsen's Somatuline Depot. It is established prescription medicine, not an experimental compound.

Hormonal

Pegvisomant

FDA

Pegvisomant (brand name Somavert) flips the usual acromegaly strategy on its head: instead of telling the tumor to make less growth hormone, it blocks growth hormone's receptor on target tissues directly. It is a PEGylated, genetically modified version of human growth hormone that acts as a receptor antagonist, FDA-approved in 2003 for acromegaly patients who do not respond well to surgery or other drugs. It is the single most effective option for normalizing IGF-1, which is why it is a key second-line therapy.

Hormonal

Macimorelin

FDA

Macimorelin (brand name Macrilen) is an orally active ghrelin receptor agonist used as a diagnostic test, not a treatment. You drink a single dose, then doctors measure how much growth hormone the pituitary releases to diagnose adult growth hormone deficiency. It was FDA approved in December 2017 as the first oral diagnostic agent for that condition.

Hormonal

HMG

FDA

HMG (human menopausal gonadotropin, sold as menotropins under the brand Menopur) is a purified hormone mix extracted from the urine of postmenopausal women. Each standard vial carries 75 IU of FSH activity and 75 IU of LH activity, with most of the LH activity coming from hCG. It is an FDA-approved fertility drug used to stimulate egg development in IVF, and it is also used, mostly off-label in the US, together with hCG to kick-start sperm production in men whose pituitary does not make enough FSH and LH. This one has decades of real clinical use behind it.
