Key Facts: HMG
- Category
- Hormonal
- FDA Status
- FDA Approved
- Clinical Status
- FDA Approved - Menopur (menotropins for injection, Ferring) for development of multiple follicles and pregnancy in ovulatory women as part of ART (initial US approval of menotropins: 1975). Use in men for hypogonadotropic hypogonadism is off-label in the US. Approved in many other countries (EU, New Zealand and others) for anovulatory infertility and ART. WADA 2026: S2.2.1 prohibits chorionic gonadotrophin and LH in males at all times; HMG contains both, so male athletes should treat it as prohibited.
- Banned in Sport (WADA)
- Prohibited at all times in males under WADA S2 (testosterone-stimulating peptides), since menotropins contain LH activity. Males only; not prohibited for females.
- Administration
- Subcutaneous injection (Menopur); intramuscular in some older male protocols
- Typical Dose
- 225 IU daily (women, IVF start); 75-150 IU 2-3x weekly with hCG (men, off-label)
- Frequency
- Daily in IVF; 2-3x weekly in male protocols
- Duration
- Up to 20 days per IVF cycle; many months (often 6-24) for sperm induction in men
How does HMG work?
FSH and LH are the two pituitary hormones that run the gonads. In women, the FSH in HMG drives multiple ovarian follicles to grow, while the LH activity (largely from hCG) supports the estrogen production those follicles need. In men, FSH acts on Sertoli cells in the testes to support sperm production, and LH activity tells Leydig cells to make testosterone, which the testes need locally at high levels for spermatogenesis. Because HMG replaces the pituitary signal directly, it works even when the hypothalamus or pituitary is shut down. That is also why it is not a 'natural' restart in the sense of turning your own axis back on; it substitutes for it while you take it.
What does the research show?
The approval rests on large, randomized trials. In the MERiT trial (731 women having IVF after GnRH agonist down-regulation), Menopur was non-inferior to recombinant FSH, with an ongoing pregnancy rate of 26.7% versus 22.3%. The US label reports ovarian hyperstimulation syndrome (OHSS) in about 6-7% of IVF patients in trials, and lists headache, abdominal pain and injection site reactions as other common effects. In men with congenital hypogonadotropic hypogonadism, HMG plus hCG has been a standard option for years: a 2020 meta-analysis in the World Journal of Men's Health summarized regimens such as HMG 75-150 IU three times a week alongside hCG 1,000-2,500 IU twice a week, and found that pulsatile GnRH got sperm appearing about 5 months sooner, though overall success rates were not significantly different. Getting sperm back this way usually takes many months, and sometimes up to two years. The evidence for HMG in men coming off anabolic steroids is much weaker; recovery guidelines and reviews in that setting mostly discuss hCG and SERMs, not HMG.
What doses have been studied?
Typical Dosingⓘ
Community experience
225 IU daily (women, IVF start); 75-150 IU 2-3x weekly with hCG (men, off-label)
75-450 IU per dose depending on protocol
Daily in IVF; 2-3x weekly in male protocols
Dosing is set and adjusted by a fertility specialist using ultrasound and estradiol (women) or semen analysis and hormone levels (men). In men, HMG is normally added after a period of hCG alone if sperm production does not start.

When and how is HMG taken?
Best Time to Take
Same time each day (IVF) or on fixed weekly days (male protocols)
Daily for up to 20 days per IVF cycle; 2-3 times weekly for months in men
Food Recommendation
With or without food
Why This Timing?
Consistent timing keeps hormone levels steady and makes monitoring easier. Many fertility clinics schedule injections in the evening, but the label does not require a specific time.
What are the side effects of HMG?
Not everyone experiences these effects. Individual responses vary based on dosage, duration, and personal factors.
- ●Ovarian hyperstimulation syndrome (OHSS), which can be serious (women)
- ●Multiple pregnancy (twins or more) (women)
- ●Abdominal pain, bloating and pelvic discomfort
- ●Headache
- ●Nausea

Frequently Asked Questions
What does HMG do?
HMG (human menopausal gonadotropin, sold as menotropins under the brand Menopur) is a purified hormone mix extracted from the urine of postmenopausal women. Each standard vial carries 75 IU of FSH activity and 75 IU of LH activity, with most of the LH activity coming from hCG. It is an FDA-approved fertility drug used to stimulate egg development in IVF, and it is also used, mostly off-label in the US, together with hCG to kick-start sperm production in men whose pituitary does not make enough FSH and LH. This one has decades of real clinical use behind it.
Related Peptides
Peptides commonly compared with HMG or used in similar applications.
Leuprolide
FDALeuprolide (brand name Lupron) is a synthetic GnRH agonist, a modified peptide far more potent than natural GnRH, and it is FDA-approved for advanced prostate cancer, endometriosis, uterine fibroids, and central precocious puberty. The clever part is counterintuitive: it works by overstimulating the GnRH system so hard that the pituitary shuts down sex hormone production. It is a real, long-established prescription drug (first US approval 1985), not a research peptide, and it carries meaningful side effects from the low-hormone state it creates.
HormonalOctreotide
FDAOctreotide is a synthetic eight-amino-acid mimic of the natural hormone somatostatin, the body's main 'off switch' for hormone secretion. It shuts down excess growth hormone, so it is a frontline FDA-approved drug for acromegaly, and it also tames the flushing and diarrhea of hormone-secreting carcinoid and other neuroendocrine tumors. This is real, approved medicine with decades of clinical data behind it, sold as Sandostatin (injectable, since 1988), Sandostatin LAR (monthly depot), and Mycapssa (oral capsule, approved 2020).
HormonalLanreotide
FDALanreotide (development code BIM-23014) is an eight-amino-acid somatostatin analog, a close cousin of octreotide, given as a long-acting deep-injection gel (Somatuline Depot/Autogel) usually once a month. The FDA approved it for acromegaly in 2007, for gastroenteropancreatic neuroendocrine tumors in 2014, and for carcinoid syndrome in 2017. Generic versions now exist alongside Ipsen's Somatuline Depot. It is established prescription medicine, not an experimental compound.
HormonalPegvisomant
FDAPegvisomant (brand name Somavert) flips the usual acromegaly strategy on its head: instead of telling the tumor to make less growth hormone, it blocks growth hormone's receptor on target tissues directly. It is a PEGylated, genetically modified version of human growth hormone that acts as a receptor antagonist, FDA-approved in 2003 for acromegaly patients who do not respond well to surgery or other drugs. It is the single most effective option for normalizing IGF-1, which is why it is a key second-line therapy.
HormonalMacimorelin
FDAMacimorelin (brand name Macrilen) is an orally active ghrelin receptor agonist used as a diagnostic test, not a treatment. You drink a single dose, then doctors measure how much growth hormone the pituitary releases to diagnose adult growth hormone deficiency. It was FDA approved in December 2017 as the first oral diagnostic agent for that condition.
HormonalTriptorelin
FDATriptorelin is a synthetic analog of gonadotropin-releasing hormone (GnRH) with a D-tryptophan swapped in at position 6, which makes it far more potent and longer-lasting than the natural hormone. As a long-acting depot it is FDA-approved to shut down testosterone in advanced prostate cancer (Trelstar) and to pause early puberty in children (Triptodur), and in Europe it is sold as Decapeptyl, including a short-acting 0.1 mg form used in IVF. In men's health circles it is known for something else: a single small dose used to 'restart' testosterone after steroid use. That idea rests on very little published evidence.
