CBL-514: The Fat-Cell Killer Being Tested With Tirzepatide

CBL-514 is an injection that makes fat cells self-destruct in one chosen spot. It is in Phase 3 for belly fat, and a new Phase 2 trial pairs it with tirzepatide to tackle stubborn fat and regain after GLP-1s. Here is what the data says so far.

CBL-514: The Fat-Cell Killer Being Tested With Tirzepatide

Why Everyone Is Suddenly Talking About CBL-514

GLP-1 drugs shrink fat everywhere. What they do not do is let you pick where. Anyone who has watched the scale drop on tirzepatide while one stubborn belly area barely moves knows exactly what we mean.

CBL-514 is built for that gap. It is an injection from Taiwan's Caliway Biopharmaceuticals that tells fat cells in one chosen area to self-destruct. And in August 2026 Caliway announced that the FDA had cleared a Phase 2 trial combining it with tirzepatide. That pairing is why it keeps showing up in our inbox (thanks to the reader who suggested it ๐Ÿ™).

Quick note before we go further: CBL-514 is not a peptide. It is a small-molecule drug. We cover it anyway, because the research community clearly wants to understand it, and because it sits right next to the GLP-1s our readers study every day.

How It Works, In Plain Words

Fat cells, like all cells, have a built-in self-destruct program called apoptosis. CBL-514 switches that program on in adipocytes (fat cells) at the injection site. Caliway's preclinical work reports raised caspase 3 and a higher Bax/Bcl-2 ratio, two markers of that internal self-destruct pathway.

The pitch is selectivity. According to the company, neighbouring skin cells, blood vessel cells and nerves are spared. That is a different approach from deoxycholic acid (Kybella), which breaks open fat cell membranes and causes local inflammation.

So the two drugs pull different levers:

What The Trials Have Shown So Far

Phase 2b (CBL-0204), belly fat under the skin. 107 adults with moderate-to-severe belly fat got up to four treatments three weeks apart, at up to 600 mg per session, or placebo. Those are trial numbers, not a recommendation for anyone. Twelve weeks after the last treatment:

  • 76.7% of the CBL-514 group improved at least one grade on the Abdominal Fat Rating Scale, versus 18.9% on placebo
  • 32% improved by two grades, versus 0% on placebo
  • Half of the treated group lost at least 150 mL of fat under the skin on MRI, something nobody on placebo did

Visceral fat, the deep fat around your organs. At EASD 2026 in Milan, researchers pooled MRI data from two Phase 2 studies (CBL-0204 and CBL-0205), 46 people in total. At week 4, visceral fat fell 10.9% on CBL-514 while it rose 4.6% on placebo. At weeks 8 to 12 the gap was 25.7 percentage points (minus 7.3% versus plus 18.4%).

That visceral result is the surprising part. The injections go into the fat under the skin, yet the deep fat around the organs went down too. Nobody has fully explained why yet, and 46 people is a small group.

Safety. The main side effects reported were mild to moderate injection site reactions. Caliway says more than 544 people have received CBL-514 across its trials.

The Tirzepatide Combination Trial

This is the trial everyone is watching. It is called CBL-0201WR: a Phase 2, randomized, single-blind, placebo-controlled study in about 120 people in the US.

It tests CBL-514 on top of tirzepatide. The main measure is belly fat under the skin on MRI. It also looks at body weight, visceral fat and lean mass.

The stated goal is two problems every GLP-1 user knows: stubborn local fat that will not budge, and fat coming back after stopping the drug. The reasoning seems to be that if CBL-514 removes fat cells instead of just shrinking them, fewer cells are left to refill later. That is a theory, not a result. The trial is there to test it.

What To Keep In Mind

  • Most data comes from the company. The numbers above come from Caliway announcements and a conference presentation, not full peer-reviewed papers.
  • The groups are small. The visceral fat analysis covered 46 people.
  • Phase 3 is still running. SUPREME-01 and SUPREME-02 are testing large-area belly fat reduction, with results expected in 2027.
  • It is not available. CBL-514 only exists inside clinical trials. Anything sold under that name outside a trial is not the real thing, or at least you cannot know that it is.

There is also a separate version, CBL-514D, in development for Dercum's disease, a rare condition with painful fatty lumps.

Our Take

CBL-514 is one of the more interesting fat-loss compounds in the pipeline. It does not try to compete with GLP-1s. It tries to finish the job they leave behind. If the combination trial shows less regain, that could change how people think about coming off tirzepatide or retatrutide.

For now, it is a "watch this space" compound. We have added a full CBL-514 profile to the database, and we will update it as the Phase 3 and combination results come in. Compare it with other fat-loss research compounds like AOD-9604 and 5-Amino-1MQ, or browse the full peptide database.
